Ensure that each specimen container is clearly marked with at least two unique patient identifiers.
Secure each specimen container tightly to avoid leakage in transit.
Complete the enclosed requisition with the patient’s demographics and insurance information.
Place the specimen inside the enclosed biohazard bag.
Place the biohazard bag inside the enclosed FedEx Clinical Pak (lab shipping bag).
Attach (if necessary) the pre-labeled and prepaid FedEx air bill.
Seal the FedEx Clinical Pak.
You can call FedEx at (800) 463-3339 or contact our Customer Service Department at (855) 4UF-SKIN to schedule a FedEx pick-up. Alternately, a pick-up can be scheduled online at fedex.com. A two-hour notice may be required for same-day pick-up.

FedEx Clinical Pak, a leak-proof envelope required for shipping bodily fluids, which allows packaging of multiple specimens into one shipping envelope
Pre-printed air bills, showing our shipping address and billing account number
Note: By using these pre-printed FedEx air bills, your specimens will be delivered to the correct destination, and we will be correctly billed for the delivery. Even though the air bills are pre-marked for next-afternoon delivery, all deliveries are made to UF Health Dermatopathology Lab in the morning.

Returns/Cancellations/Refunds
RETURNS
Money back guarantee: for goods returned within 14 days from package delivery at the customer's address. (Return policy applies in conformity with the European Law) The product must be returned in the same condition as it was received. The buyer must also include:
* All packaging such as boxes, bags, and tags
* Accessories such as cords and remotes
* All items included in a bundle
All returned products must be received in good condition. Goods will not be accepted if they are damaged or scratched. If the product is returned because the buyer changed his mind or purchased it by mistake, the buyer is responsible for paying the costs of returning the goods.
